Community Care Service Program and CHRC partnership highlighted in the April 2005 edition of Heart Beat

The Community Care Service Program (CCSP), a program managed by Visiting Nurse/Hospice Atlanta, partners with the CHRC to help an elderly patient maintan independence a little longer.

GOOD DEEDS, GOOD DESIGN
